Our Home-Based Support Services team delivers flexible, high-quality solutions tailored to each individual’s needs. We work closely with families to ensure that every person receives the support they need — in the way that works best for them.
Your Program, Your Way
At the core of our approach are Self-Directed Assistants (SDAs) who collaborate with families to create and manage personalized support plans. Services include:
- Home Visits
Regular visits to build trust, assess needs, and connect families with essential services. - Individualized Service Plans
Goal-oriented plans focused on building life skills, independence, and self-confidence — reviewed and updated annually. - Community Networking
Referrals and coordination for medical, educational, recreational, employment, and day services. - Home Modifications
Assistance with securing funding for accessibility improvements in the home. - Personal Support Workers
Guidance in finding and managing in-home support for tasks like budgeting, shopping, household management, and attending medical appointments.
