When the school bus stops coming at the age of 22, individuals with disabilities and their families are left to put together a patchwork quilt of adult services. UCP Seguin’s Building Bridges to the Future transitions team is there to help, providing guidance and expertise to families as they navigate complicated paperwork and procedures.
