RN Case Manager
DESCRIPTION
The successful candidate will assist the Director of Medical Services in the delivery of appropriate medical services, and support youth in both the Group Home and Foster Care Programs. The Nursing responsibilities will include Nursing assessments, Self-Administration of Medication Assessment (SAMA), Physical Status Review (PSR), knowledge of Medicaid Rule 116 in accordance with Medication Distribution in a group home setting. In addition, the Nurse Trainer will provide required training to all Group Home direct service staff utilizing Department of Human Services and Department of Children and Family Service curriculum and providing written validation of successful completion of on-the-Job training (OJT) hours in the areas of Basic Health and Safety and Medication Administration. In addition, the position will include a caseload of youth in care in the Foster Care Program in which the candidate will monitor youth’s health status and in foster care, provide support and instruction to Foster Parents and the youth’s medical regime.
HOURS
Primarily Monday thru Friday regular business/program hours, with scheduling flexibility to work variable hours as needed to appropriately carry out duties and responsibilities effectively and appropriate to the changing support needs of the individuals served and their families.
QUALIFICATIONS
RN with 5+ of experience of nursing experience, preferably pediatric & developmental disabilities related. Certified Nurse Training Certificate or retain the certification within 60 days of employment. Current and valid Illinois driver’s license Personal vehicle in working order. Certificate of proof of Insurance and maintenance of such at minimum amount of coverage required by law
Salary Starting: $75,000
