Case Management Coordinator – Adult Services

The Case Management Coordinator is a key player in enhancing the productivity, effectiveness, and efficiency of the QIDP team within the Department of Case Management. This role is crucial in ensuring the Agency remains compliant with IDHS documentation requirements related to participants' Personal Plans and Implementation Strategies.

ROLE AND RESPONSIBILITIES    
  Supervises, orients, trains, schedules, evaluates, and disciplines direct reports, ensuring the same for staff under their supervision. Utilizes measurable performance evaluations to support staff growth and ensure performance standards align with the agency’s mission and service objectives. 
    Promotes teamwork and effective communication by conducting regular staff meetings, reinforcing agency policies, and providing continuous informal and formal performance feedback. Ensures compliance with agency directives, policies, and quality standards. 
    Guides assigned Case Managers to ensure timely and accurate reporting, proper client record maintenance, and meaningful documentation of client progress.
   Acts as an advocate and primary service coordinator for assigned clients. Leads the Provider Support Team, oversees service plan implementation, and ensures compliance with grant-related tasks. Serves on agency committees and ensures all client needs are met in accordance with personal plans, team recommendations, and regulatory standards. 
    Assists with onboarding new staff, provides coverage for absent staff, and performs other duties as assigned to maintain effective department operations.
   May participate in special projects, research, temporary committees, mentorship programs, and cross-training initiatives. 


QUALIFICATIONS AND EDUCATION REQUIREMENTS
    Bachelor’s degree (or higher) in Social work, Psychology or a related field, as required by state regulations. 
    QIDP certified or possess 40 hours of DHS mandated QIDP classroom training.
   Minimum of two years successful work experience with person with developmental disabilities, including one year supervisory experience.
   Valid Illinois Driver's License with proof of insurance.
    Possess or acquire within first twelve months  of employment, QIDP Train the Trainer certification.
    Possess or acquire within first twelve months of employment, Safety Care certification, First Aid certification, CPR certification.


PREFERRED SKILLS
    Knowledge of theories, methodologies, technologies, and Federal and State laws and regulations pertinent to the field of developmental disabilities.
    Knowledge of the principles of English language usage, spelling, grammar, punctuation, report preparation, data collection/management and record keeping. 
    Ability to perform physically demanding tasks requiring strength, agility, stamina, and mobility, including pushing, pulling, carrying, and lifting.
    Operate personal vehicle, as necessary, for transportation needs of persons served and/or movement between program/worksites.
    Utilize and operate Agency computer equipment for word processing/data management, telephone systems and other common office equipment.
   Competency in the use of productivity tools, Adobe, Office 365 (i.e. Teams, SharePoint).
