Self-Directed Assistant/ HBS Case Manager
DESCRIPTION
To provide effective advocacy and service coordination for assigned client caseload as leader of the interdisciplinary team, ensuring that clients’ physical, social, and emotional needs and goals are identified, addressed and met, as directed by the Individual Futures Plan (IFP), ensuring appropriate service plan implementation, follow-up and monitoring is carried out and documentation completed, in accordance with policies, procedures and standards set by UCP Seguin and all applicable state (DHS) and federal regulations and accreditation agency standards, and in a manner that facilitates cross-team collaboration, cooperation and service integration across work unit, department and organizational boundaries.
QUALIFICATIONS:
Bachelor’s degree in social services, psychology or related field as required by the Ill. Department of Human Services (DHS) to achieve/maintain QIDP status. Minimum of one year of experience working with individuals with developmental disabilities.
SCHEDULE:
Monday through Friday: regular business/program hours with flexibility to work variable hours as needed to carry out duties and responsibilities effectively and appropriately.
Hourly Range: $20.30 - $22.30
