Director of Medical Coordination
Coordinates provision of all medical services and supports to persons served by UCP Seguin of Greater Chicago, ensuring the appropriate coordination of delivery of all health services, including those provided "in‑house by", as well as services provided by consultants, contractors and community health care providers in accordance with each individual's service plan and needs, and with regulations and standards governing such services as established by state and federal regulatory agencies, applicable laws and accreditation organizations; ensure UCP Sequin adult staff are trained by an RN Nurse Trainer appropriately in methods and techniques related to self administration of medication program and other applicable medical care regimes/programs and the process is monitored sufficiently to ensure effectiveness.
Selects and supervises (orients, trains, schedules, evaluates, disciplines commends, and directs daily activities) assigned subordinate staff utilizing measurable performance evaluation criteria for staff growth and ensuring that performance standards necessary to meet the needs of persons served are met, in a manner consistent with the mission and objectives of the Agency and the services it provides. Ensures that the RN Nurse trainers and LPN’s maintain their credentials with the State of Illinois and/or the Illinois Department of Human Services.
Bachelor’s Degree. Three years’ experience as an RN Nurse Trainer with a DD organization funded by the Illinois Department of Human Services. Experience coordinating medical services in a hospital/health care/ institutional setting; and two years additional professional /management experience in medical/social service field experience. Registered Nurse with state of Illinois.
RN Nurse Trainer Certification with Illinois Department of Human Services. Valid IL. Drivers License. Valid Illinois Driver’s License and Certificate of Proof of Insurance and maintenance of such at minimum amount of coverage required by law.
STARTING SALARY: $60,000
