Director of Case Management

DESCRIPTION
Oversees operation of Case Management and Home Based Departments; trains agency staff, facilitates and reviews Implementation Strategies (IS) meetings; hires, supervises, trains, leads and assists Case Management and Home Based Coordinators; monitor coordination of service plan implementation; monitor services to ensures client needs are identified, addressed and met as directed by the IS and recommended by the interdisciplinary team, all necessary follow-up is carried out, and all IS documentation is completed and in accordance with the standards set by the Agency and all applicable State and Federal governing, funding and accreditation agencies. Assures that required visits and other Home Based responsibilities and functions are handled in a timely manner; ensures all required monthly reports and billing responsibilities are completed  in a timely fashion; Assures that all required Home Based documentation is completed in accordance with the standards set by the agency and all applicable State and Federal governing, funding, and accreditation agencies.

QUALIFICATIONS
Bachelors degree in Social Services or related. Minimum of 3 years of professional/management work experience in the field of developmental disabilities and two years work experience at an administrative level. Will consider combination of education and experience. If not already QIDP approved by IDHS, you must be approved as a QIDP with IDHS within 60 days of hire and completed the QIDP training within 120 days of hire.

WORK SCHEDULE
Primarily, Monday through Friday: regular business hours with scheduling flexibility to work variable hours as needed to carry out duties and responsibilities effectively, including occasional evening and weekend scheduling availability to participate in occasional staffing's occurring outside, regular business/program hours
