Case Management/Home-Based Services Coordinator

Functions as an advocate and primary service coordinator for assigned caseload of clients, serves as leader of the interdisciplinary team; facilitates Implementation Strategy meetings; hires, supervises, trains, leads and assist Case Managers and Self-Directed Assistants (SDA); coordinates service plan implementation; ensures client needs are identified, addressed and met as directed by the Implementation Strategy recommended by the interdisciplinary team, all necessary follow‑up is carried out, all Implementation Strategy documentation is completed and in accordance with the standards set by the Agency and all applicable State and Federal governing, funding and accreditation agencies


 Selects and supervises (orients, trains, schedules, evaluates, commends, disciplines, and directs activities) assigned direct subordinate staff and ensures same for all staff supervised by direct report(s) utilizing measurable performance evaluation criteria for staff growth and ensuring that performance standards necessary to meet the needs of persons served are met, in a manner consistent with the mission and objectives of the Agency and the services it provides.

Bachelor’s degree in social work, psychology or related field as required by state regulations. Minimum of two years successful work experience with person with developmental disabilities, including one year supervisory experience. Valid Illinois Driver's License.
