Intermittent Transition Coordinator

DESCRIPTION
To provide effective development, monitoring, training and oversight which ensures behavior plans, program activities and environment are consistent with goals and objectives, promotes the health and safety and growth in residents’ acquisition of daily living and interpersonal skills needed to function as independently as possible in the community in accordance with applicable certification and DHS licensing requirements and standards. Develop Behavior plans which focus on skill acquisition, coordinate all community and leisure activities, personal and grocery shopping, medical appointments and electronic medical tracking, meal planning for assigned CILA site in addition to job developing.

Selects and supervises (orients, trains, schedules, evaluates, disciplines, commends, and directs daily activities) assigned subordinate staff utilizing measurable performance evaluation criteria for staff growth and ensuring performance standards necessary to meet the needs of persons served, and respond to Agency pager within departmental designated timeframes.

QUALIFICATIONS
Bachelor’s degree from an accredited school and status as a Level II Behavior Analyst, National Provider Identification Number (NPI) required at the time of hire. Minimum of one year of experience with children/adults with intellectual/developmental disabilities. Valid Illinois Driver's License & Insurence

WORK SCHEDULE
40Hrs a Week/Flexible, Schedule finalized at the time of hire.
